You get the alt-fire mode (bring the sights to your eyes) as well as recticle sight for the crossbow and Bullpup. Those are the only weapons I've tried!!

There is melee exterm, one of the perks is a beserker or something which makes him kick arse. And Friendly fire is a server setting, but most servers have it turned off I've found.

Sweet. One last question. These perks, what's the deal with getting/changing/activaying them? Which ever weapon you choose adds to your experience against each specific perk. Experience is what governs the break points for each perk level. So to gain Level 1 Field Medic you have to heal 2000 health on other team players. From what I've seen you are always gaining expereience no matter which perk you currently have selected.
IIRC you can select your perk at the start of the map and after each wave. If, like Field Support you have two stats that require experience in order to progress. As long as your playing your experience contains to accummulate even passed its current threshold... which helps you attain higher levels. (hope that one makes sense!! bit brain dead at the moment)

You get perks by killing monsters (30 head shots with the Handcannon, or healing 20,000 health) and such, there are six in total that give you different type of boosts. Each one of these six perks can be upgraded 5 times i think.
So, i've gotten to lvl 2 of the Support Specialist Perk, which means i can carry 15% more, 20% more damage with shotguns, can weld doors shut 25% faster and such.
They're a nice addition, but as Bloke said, not absolutely essential to survive.
