We got Silver Bullets on chapter The Terminal of campaign Dead Air:

We also reached the gold standard on Blood Harvest Level 2, I can only show a picture of the silver one though:

Survival mode pretty much annihilated any chances of me getting 100% L4D achievements, Valve be damned!
